Supporting people to get out of the hospital and safely back into their own homes is key to their rehabilitation and vital to improving hospital capacity and resources.
Technology can play a key role in enabling hospital discharge and giving people the confidence to be back at home. As was discussed at the TSA – The voice of Technology Enabled Care event in Leeds this month, a holistic view of care leads to the best outcomes for the individual.
Nottingham On Call
Nottingham On Call have had a foot forward with digital for a few years now and are finding new and improved ways to integrate digital TECS into their work. In partnership with social care, they have ensured safe and timely discharges from hospital, to both get people back to the comfort of their own homes, and to free up much needed bed space in hospitals.
How do they do it?
“The patient is provided with a pre-set alarm, which has already been programmed to work as soon as they get home. The patient or their support network can plug in the alarm to listen to the audio guide, taking them through the set-up and testing process, so they can be reassured that the alarm has been set up right and is working. This means the patient can be safe and supported from the moment they arrive home and the alarm is plugged in, rather than being delayed in hospital whilst an installation is booked in.”

“Nottingham On Call’s GO! alarm has changed my life. I now go out just like I used to before but now I feel safe and protected if I fell over or felt unwell. Thank you for providing this.” – Doreen, Nottingham On Call Customer
